Pre-K, Kindergarten, & 1st Grade Registration 2013/2014
New Students
Please bring the following items:
The following documents are required for registration:
- Child’s Birth Certificate (must be certified copy)
- Child’s Social Security Card
- Child’s Current Immunization Records- Up-to-date
- Proof of Residency (current utility bill – water, gas, electricity—with name and address or signed Lease Agreement, builder’s Contract with completion dates)
- Driver’s License or Photo ID of parent or legal guardian enrolling student or valid picture ID
- (Pre-K Only) Income verification - TANF or Food Stamp documentation letter and the last two months of paycheck stubs for all members of the household
- Military Documentation, if applicable (Pre-K only)
- Documentation from the DFPS showing verification of eligibility to participate in PK program, if applicable (Pre-K Only)
2013-2014 Pre-K Eligibility Information
To enroll in prekindergarten for the 2013-2014 school year:
1. Your child must be 4 years old on or before September 1, 2013.
2. Your child must be:
Income eligible),
or
Limited English speaking (your child will be given a Language Proficiency Test),
or Homeless as defined by 42 USC 11302,
or Child of an active duty member of the U.S. armed forces or who was injured or killed while serving on active duty,
or placed in foster Care of the Department of Family and Protective Services (DFPS) following an adversary hearing.

Tarjeta del Número de Seguro Social
Cartilla de Vacunación Actualizada
Comprobante de domicilio (recibo actual o reciente – de agua, gas, electricidad—con su nombre y dirección o bien el Acuerdo de Renta, o Contrato de la constructora con las fechas de terminación)
Licencia de Conducir o una identificación con fotografía del padre o tutor legal que inscribe al alumno o una identificación con fotografía válida
Verificación de ingresos (Talón del recibo o cheque de pago del mes anterior, TANF o carta de documentación de Estampillas para Alimentos)--PK only
Documentación militar, si aplica--PK only
Documentación del DFPS mostrando la verificación de elegibilidad para participar en el programa de PK, si aplica--PK only
Para inscribirse en pre-kinder para elaño escolar 2013-2014:
1. Su hijo debe tener 4 años cumplidos el,o antes del, 1º de septiembre de 2013 (Pre-Kínder)
2. Su hijodebe:
· Serelegible de acuerdo a su rango de ingresos, o
· Tenerun nivel de Inglés Limitado (su hijo tomará una Prueba de Aptitud del Idioma), o
· Carecerde un hogar (homeless), o
· Serhijo de un miembro en servicio activo de las fuerzas armadas de Estados Unidoso que ha sido herido o caído estando en servicio activo (Pre-Kínder solamente).
· Estarbajo el Cuidado Adoptivo del Departamento de Servicios para la Familia y deProtección (DFPS, por sus siglas en inglés) posterior a una audienciaadversaria.
